Your Responsibility for Maintaining Posters at Your Site
Federal, State and Local laws and/or funding and best practices guidelines dictate that NKY Health post certain notices in location available to employees, applicants and/or certain other populations. In many cases, they also dictate the size, color and/or layout of the posting.
NKY Health provides an "All-In-One" poster that is compliant with Federal and State requirements to each physical site within the district.
As a site supervisor at NKY Health, you are responsible for ensuring that your site complies with these requirements by ensuring that the postings are visible as required, either by posting the all-in-one poster or by making the individual posters available. All posters must be visible (and not stacked on top of each other).
Human Resources (HR) will ensure that applicants scheduled for an interview receive these as part of their electronic interview packets.

Your Responsibility for OSHA 300 Forms
As a site supervisor at NKY Health, you are responsible for:
Recording any recordable work-related injuries or illnesses involving employees according to OSHA's instructions on the OSHA 300 form Human Resources provides as soon as you become aware of the incident.
Consulting with Human Resources for clarification of any of OSHA's instructions that you do not understand.
Notifying Human Resources of any "Privacy Concern" cases.
Preparing and sending the OSHA 300A Summary form as instructed by Human Resources (usually at the end of December or first week of January).
Posting the final OSHA 300A Summary form between February 1 and April 30 each year. (Human Reosurces usually sends this at the end of Janaury.)
How to Prepare and Post OSHA 300 Forms
Please do all of the following by the end of the first week in January:
Please be sure that the recordable cases for the previous calendar year are listed on OSHA's Form 300. (When in doubt, please check with Human Resources.)
Tally each of the the columns on OSHA's Form 300 and enter the totals in the "Page totals" row at the bottom. Use 0's when applicable. Please do not leave any blank.
For each numbered item in Column A on OSHA's Form 300, ensure Column B contains one of the following: an employee's name, "privacy concern" or "N/A."
Pull the numbers from your tally on on OSHA's Form 300 over into the corresponding "Cases," "Days," and "Injury and Illness Types" fields on OSHA's Form 300A. Use 0's when applicable.
Leave the "Establishment Information" section of on OSHA's Form 300A blank.
Scan both forms into a single .PDF and email them to hr@nkyhealth.org.
Once the payroll is processed and Human Resources has correct numbers, Human Resources will complete the "Establishment Information" information and email it to you to post between February 1 and April 30.
Human Resources will also send you a new blank form to use for recording starting January 1.
