ALL incidents involving work-related injuries and illnesses must be reported immediately.
ALL incidents of reasonable suspicion of impairment due to the influence of drugs or alcohol must be addressed immediately.

EMPLOYEES
Employees must report incidents to their supervisor and to the hotline at KACO Claims center using 866-367-5226. They should notify the KACO Claims Center that the supervisor will complete Form IA-1 First Report of Injury form.
SUPERVISORS
As soon as they learn of an incident, supervisors must follow the Incident Reporting Procedures described in the Incident Management section of NKY Health's Safety Manual.
Fatalities, hospitalizations and certain severe injuries to employees must be reported to OSHA within 8 hours as per OSHA's Reporting Requirements.) Supervisors are responsible for:

Notifying the site manager of any incident involving her/his site.
Notifying the head of Human Resources of any incident involving an employee.
SITE MANAGERS
Site managers (Clinic Managers for health centers; Human Resources for the District Office) must record certain work-related incidents to the OSHA 300 form as per OSHA's record-keeping guidelines.
