What EMPLOYEES must do:
Employees must report incidents to their supervisor and to the hotline at KACO Claims center using 866-367-5226. They should notify the KACO Claims Center that the supervisor at NKY Health will complete Form IA-1 First Report of Injury form and that KACO does not need to complete the same form.
What SUPERVISORS must do:
As soon as they learn of an incident, supervisors must follow the Procedures for Reporting Employee Incidents described in the Incident Management section of NKY Health's Safety Manual.
Fatalities, hospitalizations and certain severe injuries to employees must be reported to OSHA within 8 hours as per OSHA's Reporting Requirements.)
Supervisors must:

Notify the site manager of any incident involving her/his site.
Notify the head of Human Resources of any incident involving an employee.
What SITE MANAGERS must do:
Site managers (Clinic Managers for health centers; Human Resources for the District Office) must record certain work-related incidents to the OSHA 300 form as per OSHA's record-keeping guidelines.
