Appointments for Rechecks
The appointment system: Appointments for rechecks are now scheduled through our Lindsay scheduling system. To enter a Vet Recheck or Tech Recheck appointment request go to the following link and follow the prompts. The system will show you what times are available, and request information about you and the patients.
Staff will block out times that a vet or tech will not be available. The system will also block out the times for appointments that are already scheduled.
You will be able to schedule a vet check or a tech check. If you are bringing in multiple patients that need different types of checks, you can schedule a vet check and a tech check for the same time if an appointment slot is available (you would need to make two separate appointment requests). You can book a maximum of 3 patients in a single one hour appointment slot.
To allow for staff scheduling, there is a minimum advance notice for scheduling or canceling an appointment:
Vet Checks: require at least 16 hours advance notice to schedule an appointment.
Tech Checks: require at least 10 hours advance notice to schedule an appointment.
Cancel/Reschedule: Both types of checks require at least 4 hours advance notice to cancel or reschedule an appointment through the system. The appointment confirmation email contains a "Cancel" link that can be used to cancel up to 4 hours before the appointment. If you need to cancel or reschedule within 4 hours of the appointment, please call the hospital.
When to schedule a recheck appointment: Please try to schedule recheck appointments for as close as possible to when you were told that the patients need them.
However, do not fret if it is off by a day. Go ahead and book an appointment for the following day and email the hospital to inform us of the discrepancy so we can make a note/change in WRMD.
If you cannot make a recheck appointment within one day on either side of the requested recheck (e.g. one day earlier or one day later), please email the hospital so we can provide a new recommended recheck appointment. Some things (such as wound suture rechecks) are OK to wait a day or two after the requested day, but other things (such as corneal ulcers, prolapse repairs, etc.) should come back earlier rather than later. If there’s any question please just send the hospital an email.
If a delayed recheck occurs at the end of a medication series, please email the hospital to see if the medication should be continued until the recheck appointment.
Filling out the "Provide your information" page:
Any question with an asterisk (*) must have an entry in the answer field.
When you get to the end of the page, if you click "Continue ->" and it doesn't move on the the next page, you have a required answer field that is still blank. Scroll back up the page, the offending question will be highlighted in red.
The Comments field is limited to 500 characters. So while you do not need to be overly brief, your comments do need to fit within this field.
Confirmation emails: You will receive an email with your booking request information. If a technician confirms the appointment you will also receive a confirmation email. If the appointment is declined you will receive a message with the reason, and you can book an alternate appointment. When you get your booking request or confirmation emails, do not attempt to close the page by clicking on "Cancel", this button is used to Cancel the appointment.
Not for emergencies: This scheduling system should NOT be used to schedule appointments for medical emergencies. If there is a medical emergency with your patient(s) that you think cannot wait for a recheck appointment, please CALL US AS SOON AS POSSIBLE (but before leaving your house). We may be able to provide assistance over the phone, and we may need to inform other homecare volunteers that their rechecks need to be rescheduled.
We are implementing this appointment system to better optimize workflow and ensure that everybody's time is respected. Because we are also an emergency room and the unforeseen may happen, circumstances may dictate that your appointment may need to be changed, or we may request a drop off. We will notify you of potential time issues as soon as we are aware.
All appointment requests must be entered through the Lindsay scheduling system. If you have technical issues, please call or email at least 48 hours in advance of the appointment and let us know the nature of the problem. (hospital@lindsaywildlife.org | 925-627-2960)
Dispositions
All homecare teams and homecare volunteers please make sure your homecare records are kept up-to-date, especially final dispositions (released, died, etc.). There are multiple ways you can report dispositions:

If you fill out and submit this webform, you do not need to submit the paper form. In fact, it makes updating WRMD easier as we can cut/paste release locations, and don’t have to interpret handwriting on the paper forms.
You can also:
Send it to your Homecare Team record manager, if your team has WRMD access. If your team requires final dispositions to be tracked through the team, please continue to do so.
Fill out the paper Homecare Record and return it to the basket above the printer in the Volunteer Room.
Email all the information to hospital@lindsaywildlife.org.
Enter it into WRMD---only if you are trained and have been granted access.
Forms for Use at a Satellite Site
These forms may be needed during operation of a Satellite Site.
Incident Report Form. An Incident Report Form must be filled out for any injury, including bites, that occurs during Lindsay activities, even off-site. This link is for the on-site version that must be printed, filled out, signed, and dropped off at Lindsay. We are working on a digital satellite care version, which will be posted here when ready.
Permits and Agreements
The regulatory permits and agreements required for a satellite site are in the Permits and Agreemnts section below. The documents are the same whether you are starting a new site or maintaining an existing site.

Permits and Agreements to Keep at Satellite Site
Volunteer and staff Satellite Sites must maintain current copies of all applicable permits and agreements to legally provide wildlife rehabilitation Satellite Care. Some of the documents are printed out and kept at the Satellite Site, some must be signed and returned to the Lindsay.
Memorandum of Understanding (MOU) between Lindsay and California Department of Fish & Wildlife (Expires 12/31/23) -- read and retain a copy at the Satellite Site.
Lindsay Wildlife Experience and the California Department of Fish and Wildlife have an arrangement (Memorandum of Understanding) that allows Lindsay Wildlife Experience (Lindsay) to rehabilitate wildlife. The MOU requires that "every paid staff member or unpaid volunteer of a wildlife rehabilitation organization who physically handles wildlife shall read [the] MOU and sign a document affirming he or she understands and shall abide by the terms and conditions of the MOU". Lindsay must keep a copy of your signed agreement (the Lindsay Copy). If you do Satellite Care, a signed copy of this agreement must also be kept with your copy of the MOU (the Satellite Care Copy) and both the MOU and agreement must be kept available for inspection at your Satellite Care site. The MOU covers only those animals officially accessioned into the Lindsay Wildlife Experience rehabilitation program. You must read the MOU whenever it is renewed (every 3 years).
MOU Agreement Letter-Satellite Care Copy (Expires 12/31/23) -- read, print, sign, and keep with your Satellite Site permits. A new Agreement to abide by the provisions of the MOU must be signed whenever the MOU is renewed (every 3 years).
California Department of Fish & Wildlife Rehabilitation Permit -- print and keep with your homecare permits, also carry a copy in your vehicle during transport. Print a fresh copy whenever the MOU is renewed (every 3 years).
US Fish & Wildlife Migratory Bird Rehabilitation Permit -- print and keep with your Satellite Site permits if you are a homecare provider for any bird species. Print a fresh copy whenever the Permit is renewed (every 5 years).
Agreements to Fill-Out and Return to Lindsay
These documents need to be read, filled out, and a copy returned to the Hospital Volunteer Manager (you can also retain a copy for your records). These are forms that you can fill out on the computer, so you can click the check-box to indicate agreement, type in your name, save, and return a digital copy to: hospitalshifts@lindsaywildlife.org. You can also print them out, sign them, and paper-mail or drop them off at the hospital office.
MOU Agreement Letter-Lindsay Copy (Expires 12/31/23) -- read, fill out, and return a digital copy (you may have already done this as part of the new volunteer paperwork) You must read and agree to abide by the provisions of the MOU whenever it is renewed (every 3 years).
Lindsay Wildlife Experience Homecare Agreement -- read, fill out, and return a digital copy. Only needs to be completed once, when you start as a Satellite Care volunteer.
Reference Material for Satellite Sites
You should be aware of the information in these documents, but you don't need to print them out and there is nothing to sign or return.
California Code of Regulations, Title 14 Section 679, Possession of Wildlife and Wildlife Rehabilitation -- read (you don't need to print it out), be aware of the sections of California law regarding wildlife rehabilitation.
Minimum Standards for Wildlife Rehabilitation (NWRA/IWRC) -- for your reference, you don't need to print it out.
The "Minimum Standards for Wildlife Rehabilitation" manual is published by the National Wildlife Rehabilitation Association and the International Wildlife Rehabilitation Council, and outlines the standards of care and caging for wildlife rehabilitation. The MOU (described above) specifies that the Permittee (Lindsay) may possess and provide care for sick, injured, or orphaned wildlife pursuant to California Code of Regulations, Title 14 Section 679 (see below), and the "Third Edition 2000 National Wildlife Rehabilitation Association/International Wildlife Rehabilitation Council Minimum Standards for Wildlife Rehabilitation". Title 14 Section 679 also directly references the "Minimum Standards for Wildlife Rehabilitation, 2000, Third Edition manual". A copy of Minimum Standards for Wildlife Rehabilitation (3rd Edition, 2000) can be found at the link below as a reference. Note that the MOU also states: "Wildlife caging shall comply with the 2000 Wildlife Rehabilitation and Care Standards".
Following the Third Edition of Minimum Standards for Wildlife Rehabilitation published in 2000, there have been two revisions, Fourth Edition in 2012 and Fifth Edition in 2021. To ensure that Lindsay is in compliance with future updates to the MOU or Title 14 Section 679, all new enclosures/caging must satisfy the updated standards. A copy of Minimum Standards for Wildlife Rehabilitation (4th Edition, 2012) can be found at the link below as a reference.

Satellite Care Enclosure Database
With the 2017 permit renewal rehabilitation centers must report details of homecare enclosures at each satellite (homecare) location. This includes dimensions, a photo, and the species housed in that enclosure. The webforms below allow homecare volunteers to report enclosure information so we can update the permit-report database.
Enclosures to Add to Database
Use these webforms to add enclosures to the database. If the enclosure does not yet have a Lindsay Wildlife Hospital ID number then it must be entered here.
Why are there two versions of the form? Google Forms has a nifty feature that lets the person filling out the form upload an image file as a part of the form response. This is the best way to help us keep the proper picture attached to the enclosure description. However, the upload capability requires you to log in to the form with a Google/gmail account. If you have a Google account please use the Upload Picture version of the form. Otherwise, please use the Email Picture version of the form and email the picture to us at: hospitalshifts@lindsaywildlife.org .
Transferring Enclosures to a New Location
When an enclosure with a Lindsay Wildlife Hospital ID number is transferred, use this form to report the new location. This is a much simpler form, as the dimension, description, and picture are already entered in the database.
