All new hires at Sunny Haven Residential Services are required to complete a Tuberculosis (TB) Assessment as part of the pre-employment health screening process. This assessment helps ensure the safety of both employees and the individuals we support by preventing the spread of tuberculosis, a potentially serious infectious disease.

All new hires must complete a TB screening before starting work.

Employees returning after an extended absence may be required to complete a new TB assessment.

Annual reassessments may be required based on state health regulations and individual risk factors.
Step 1: TB Risk Assessment Form
Employees must complete a TB Risk Assessment Questionnaire, which evaluates potential exposure and symptoms.
This form must be filled out honestly and completely and submitted to HR.
Step 2: TB Test (If Required)
If no risk factors or symptoms are identified, no further testing is needed.
If the assessment indicates possible risk factors (e.g., past TB exposure, travel to high-risk areas, symptoms such as persistent cough or unexplained weight loss), a TB skin test (PPD) or blood test (IGRA) will be required.
If the TB test is positive, a chest X-ray and medical clearance will be required before starting work.

Failure to complete or update TB screening as required may result in a delay in employment or removal from the schedule until compliance is met.
