Class Reimbursement Form Instructions

Climb leaders should use this form to request reimbursement for classes taken to maintain climb leader certification.  The form will send reimbursement requests to Finance.  Someone will then ask CC for authorization.  

The form is confusing. Please fill it out in the following way:

• Type of Expense: Committee

• Are you the committee chair: No

• Select your committee name: Climbing

• Department (Class): Climbing   ← YES, THIS IS NON-INTUITIVE. PLEASE FILL OUT THE FORM THIS WAY.

• Category: Leader Training

• Amount: whatever you paid

• Date: when you took the class

Attach relevant receipts.

THERE IS NO PLACE ON THE FORM THAT ASKS WHAT CLASS YOU TOOK.  YOUR RECEIPT SHOULD SHOW THIS.
