The Heart Improvement Project (HIP), is a FREE, 2- year project, under the Win With Wellness Initiative, that provides wellness programs to lower cardiovascular disease risk in Stephenson and Carroll counties.

Heart-to-Heart (HH) involves 8 short, interactive lessons on physical activity, healthy eating and health literacy. It is a free program to promote healthy living. Presentations are held in person at participating work sites or community organizations, as well as online. The HH online lessons are available below.

Each lesson contains three parts:

    1. A video presentation

    2. A learning activity

    3. An opportunity to apply what you learned to your life
