The time leading up to June 30th is extremely busy for the department with the large volume of reference requests, ordering certificates, and prepping for the onboarding of our new R1s starting on July 1st.
Please try to give yourself enough time between completing the program and starting work somewhere - once a reference request form or the document to sign you off to CFPC leaves our office, it is out of our hands how long it will take on the other end to process.
CPSA Reference Request Form: Please send your reference request form as early as possible (around April) and the first section filled out including your CPSA tracking number to FMPA (fmpa@ualberta.ca) for Program Director completion and signature. DO NOT WAIT UNTIL THE LAST MINUTE. Early submission will give us enough time to ensure that all of your program requirements are met so that Dr. Morros can fill out and sign the form without delay. FMPA will submit your reference request form to the CPSA with a verification of residency letter.
CFPC Sign Off: The CFPC sends the program a Certificate of Confirmation of Completion of Training Certification Examination in Family Medicine document. This document attests that:
the resident name has achieved competence in the six essential skill dimensions and the phases of the clinical encounter, throughout the seven CanMEDS-FM roles, over a sufficient sample of the priority topics, themes, core procedures, and competencies, as defined by the Evaluation Objectives and CandMEDS-FM, and
has completed the residency training program.
This document requires the Program Director's signature followed by the Dean's signature to be submitted. Though we do everything in our power to ensure prompt turnover to avoid delays, it can sometimes take several days to get the Dean's signature on this document before we can send it back to the CFPC. Because of this, it is imperative that you ensure all of your program requirements have been completed and submitted in advance of your end date to avoid any delays. It is beyond our control once the document leaves our office how long it may take to process.
The program WILL NOT sign off on this document until all program requirements have been met and applicable documents submitted. What is the process for being signed off and able to work?
Once all mandatory requirements are met, Dr. Morros will sign off on the CFPC document.
The PD signed CFPC document is then sent to PGME for the Dean's signature.
Once the Dean has also signed the CFPC document, it is then emailed to the CFPC to "sign you off to the college".
To receive your license, send your CPSA/CPSBC/etc (whichever province you plan to work) Reference Request form to FMPA with your information filled out on the top section including the tracking number.
You need to be signed off by the college to get your license, but you can start the process early and avoid delay by sending your Reference Request form to FMPA in advance.
Dr. Morros WILL NOT fill out this form until your program requirements are met - when they are, she will review your file and complete the form.
FMPA will email your Reference Request form back to the CPSA with an accompanying Resident Verification Letter. You will not be copied on this email, but we can inform you when it has been sent if you request.
