Complex Care SPA - Service starting 20th November 2023
This service can offer advice, admission avoidance support, signposting and urgent response. Simply call 0300 123 4868 and select option 3.
The service is available 8am to 8pm, 7 days per week. Referrals are accepted from all health and social care professionals in Lincolnshire.
Service is for patients where support is needed in the community but unsure which service to send them. The service is intended to minimise the bouncing around of services and will triage to the most appropriate team.
The clinically led SPA will be available to all health and social care professionals, including GPs, EMAS and social care
This would typically be patients who don’t fit easily into the criteria for any one service, but who clearly need help in a timely manner. This could range from moving and handling support to input from community nurses or the IV team, or a combination of them all.
