How to add reasonable Adjustment flag to patients records
The patient needs to complete the form - this can be printing from either the QOF + Enhanced Services template or the All reception Templates
Once complete the Reasonable Adjustment, Communications & Accessibility Template needs completing (Only complete the Digital Flag consent if the patient ticks the consent on the form)
Scan the form in the patients records
