LCHS provides care for patients who require clinical input from either planned or urgent care services coordinated by the Operations Centre. Planned care is provided by our community nursing, therapy teams and specialist services in the patients home, community hospitals or in care home beds. Urgent care is provided via our Clinical Assessment Service supported by teams in our urgent care bases and by urgent care home visiting practitioners by dialling 111.
The Trust will cease to use fax machines with effect from Friday, 20 March 2020. This is a mandate set by the health secretary that NHS organisations will be required to use modern communication methods, such as secure email, to improve patient safety and enhance data security and cyber security.
To our access our services referrals are made to the Operations Centre by social and health care professionals and for a small number of services self-referrals are also accepted.
If you require access to one of our services where self-referral is not an option please contact your GP to discuss your requirements.

Electronic Assistive Technology Service Referral Information
AAC Service
Referrals to the AAC Service (Communication Aids) are usually made through a speech and language therapist, however they can come from any health or social care professional. In order to make a referral to the service please see this page http://www.eastmidsaac.nhs.uk/clinicians/communication-aids on the Electronic Assist Technology (EATS) website and complete the form at the bottom of the page. Each referral is considered in the weekly triage meeting against the service specification and the team will be back in touch with you following this meeting to make arrangements for an initial assessment.
Environmental Controls / Computer Access
Referrals to the Environmental Controls Service are usually made through an occupational therapist, however they can come from any health or social care professional.
