Rapid Access Chest Pain Clinic Referral Criteria
Referral Criteria:
Exertional chest pain suspected to be ischaemic in origin
(Example: typical areas chest, neck, jaw, arms, upper back on exertion, resolves <5 minutes at rest)
Age >40 unless exceptional circumstances (please detail this in the reason for referral)
Recent bloods taken (last 6 months). Must include: FBC, U+E, TFT, LFT, Lipids and Hba1c
If shortness of breath only and no chest pains please request a CXR/ BNP and consider other causes too
Exclusions (all answers below must be 'No')
Does the patient have uncontrolled heart failure?
Do they have suspected aortic stenosis? (if so request an echo and if AS confirmed refer to General Consultant Cardiologist)
Do they have an uncontrolled arrhythmia? (if so refer to Palpitations clinic)
Under current follow up in cardiology outpatient clinic
Important Information:
Referrals without ECG attached, or recent bloods will be rejected
Palpitations Clinic Referral Criteria
Referral criteria:
Symptoms of palpitations (more than a one off episode)
AF that is a potential candidate for DCCV and or ablation (if for rate control only for the GP to manage and up-titrate medications)
Evidence of pre excitation
Evidence or suspicion of SVT that may need referral for ablation
Recent bloods taken (last 6 months). Must include: FBC, U+E, TFT, LFT and Lipids
Exclusions (all answers below must be 'No')
Under current follow up in cardiology outpatient clinic
No evidence of syncope with the palpitations (please refer to general Cardiology clinic)
Important Information:
Referrals without recent bloods will be rejected
Consider requesting a 24hr tape +/- echo if appropriate due to long waits and clinic appointments
Attached any recent 12 lead ECG/24 ECG recording to referral
Rapid Access Heart Failure Clinic Referral Criteria
Referral criteria:
Presence of suspected heart failure symptoms and a NT-proBNP level of >2000ng/L
Suspected heart failure symptoms and a history of a previous MI.
Presence of suspected heart failure symptoms and a NT-proBNP of >400ng/l
Recent bloods taken and include: FBC, U+E’s, TSH and NT-proBNP
Recent 12 Lead ECG
Physical Exam Details needed: Pulse / BP / Presence of a Heart Murmur
