· Patient must have a diagnosis of a cardiovascular disease (Heart disease, Stroke, TIA, PAD)
Plus one of the following:
· Despite appropriate medication, unable to achieve target lipid profile in patients treated for secondary prevention (Targets: <1.8mmol/l (<1.4mmol/l if also diabetic)
· Patient intolerant of lipid-lowering medication
· Patient has had other significant adverse effect to lipid-lowering medication
Excluded from Lipidology Nurse Clinic: Please use ULHT advice and guidance or refer to their local hospital lipid service
· Suspected Familial Hypercholesterolaemia
· Severely elevated triglycerides (>10mmols/l)
