who has been affected by any form of sexual violence at any point in their lives,
and is willing to engage in support/therapy relating to these experiences.
Any form of sexual violence includes: sexual assault, rape, sexual abuse, incest, sexual domestic violence, trafficking and sexual exploitation, female genital mutilation, ritual abuse, forced marriage, crimes in the name of honour, sexual intimidation/coercion/harassment, whether physical or verbal.
Email and telephone support, empowerment and wellbeing support (face to face), trauma focused therapy, group peer support, therapeutic yoga, and we are soon to be launching our ISVA service. Our assessment process establishes which form of support is best for each individual.

CARE HOME LIAISON SERVICEADVANCE CARE PLANNING
The Care Home Liaison Nurses will create an advance care plan to document the patients’ wishes for their end of life care. If the patient does not have the mental capacity to participate in the process, their next of kin will be invited to discuss this aspect of care in the patient’s best interests. Referrals can come from any source and involves just a phone call (Mobile 0791 921 8629) giving us the details of the patient ie name, date of birth and care home they are based at. Topics for discussion can include preferred place of care, resuscitation, use of antibiotics and whether invasive investigations/ treatments are appropriate towards the end of the person’s life.

Parkinson's Nurse Specialist Service
Children under 16
Patients not registered with SWL CCG practice
Adults not diagnosed with Parkinson's
Patients with Parkinson's secondary to medication
Advance care plans are shared with the care home, GP and are filed in the patients’ medical records. A copy is also given to relatives if they are involved in the discussion.
Telephone and emails access to the Specialist Nurse for those diagnosed
Specialist patient assessment to look at the needs of each person with Parkinson's, Family and carers
Provision of Specialist Nurse clinics in the community
Partnership working with other professions involved in caring for a person with Parkinson's
Support to monitor and review the effectiveness of Parkinson's medication
Education to individuals, groups and multidisciplinary health and social care professionals to promote a greater understanding and awareness of Parkinson's
Community Paediatrics
Pre-school age children and young people should be re-directed to speak to either their Health Visitor or Early Years Setting tel: 01522 843000
Children & young people out of education can be re-directed to speak to their Health Visitor or CYP Nurse via the same tel no.
GP's will still make referrals for CYP moving into the area with a known need/diagnosis requiring follow up and babies and toddlers with complex medical health needs including genetic syndromes.
