Referrals can be made by a patient and/or carer and any health and social care professional
The urgent community response is a new service that is here to compliment existing pathways, by seeing those patients in the home environment and preventing onward conveyance to A&E.
Referral Information and Criteria
18 Years old and registered (or temp registered) at a Lincolnshire Practice
Exclusions:
Anyone that does not need a 2 hour response or is not at risk of hospital admission and can be managed by existing services
Anyone under 18
Pregnant women
a 999 emergency
Not registered or temp registered with a Lincolnshire practice
Examples
The 0-2 hour service is aimed at patients known or suspected to be suffering from some criteria such as and not limited to:
Fall or collapse - where there is no apparent acute injury
Found on floor - Where the individual has been assisted off the floor and required crisis response i.e. LIVES
A sudden loss of mobility ("off legs")
Sudden loss of function
Sudden new acute confusion (Post Medical Review)
Requirement for equipment needs (To prevent harm/avoid hospital admission)
End of Life care (In collaboration with existing Palliative Pathways)
