A. Specify your goals/objectives: Reflect upon your job description, knowledge of student performance, and the goals for school improvement established by your district and/or school to determine at least three (3) Performance Goals you hope to achieve this year. You may develop more than three goals if you wish. The first two goals must be in the areas of technology and utilization of data. The last goal will be dictated by your personal choices for professional growth. Your progress in achieving these goals will be reflected in your evaluations and "Section D: Assessment of Results" will be completed as part of your Summative Evaluation Conference. At that time you and your administrator will each sign this form indicating the level of progress made on each goal you articulated.
- Goal #1: [For Technology Implementation] I will use my knowledge of technology to help prepare my students to effectively use the proper technology tools to successfully complete their electronic portfolio requirement for eighth grade.
- Goal #2: [For using data, including assessment data from NECAP’S and NWEA’S, to guide instruction] I will use the curriculum ladders to determine the achievement level of each of my students in two specific areas on the NWEA mathematics test: 1) Numbers & Operations and 2) Measurement. Using this information, I will devise a variety of activities focused on helping students move up at least one level on each ladder.
- Goal # 3: I will take the necessary steps towards obtaining both my Masters of Education in Mathematics and Computer Technology Education by the end of the summer of 2008. I will also fulfill at least 50% of the Alternate IV requirements, to ensure completion by 2009.

- Work with the technology coordinator to assure that each student is able to save documents to their portfolio folder.
- Teach students the process for saving to their portfolio folders.
- Assign a variety of technology projects using the various tools available therefore allowing them the opportunity to learn those tools.

- Enter onto the ladders the names of the students performing at each level of the ladders.
- Consult the teacher resources accompanying the student textbook for suggested activities that focus on the skills the students need to learn in order to move up the ladders.
- Deliver lessons and provide activities that offer focused instruction and practice on each of the necessary skills.
- Goal # 3:
- Attend courses at Plymouth State University per Masters Degree Requirements and obtain an 85 or better in these courses.
- Complete Alternate IV paperwork with the assistance of my mentor.
- Work with the Principal, Superintendent and my peers to achieve the Alternate IV requirements not covered by the courses at Plymouth State.
- Maintain necessary records to show proof of Alternate IV achievement.
