2 Self Assessment, Student/Staff Feedback and Goals

CPS Educator Plan Form Self-Assessment & Goal Setting is completed by educators on one year plans or in their first year of a two year plan and shared with evaluators by October 15. In the 2022-23 school year, Educators on their second year of a two year plan update their goals

Educators in year 2: You need to start from the beginning. If you have not yet met your goal you can keep it, or you can update it. The Self Assessment and Student/Staff Feedback sections of the form can be the same as year one.

Evaluator must meet with Educators in their first year of practice and Administrators in their first year in a position by October 15 to assist in the self assessment and drafting goals which must include induction activities.

Self Assessment:

  • Done by the Educator, not the Evaluator.

  • Examine evidence of student learning, growth and achievement.

  • Use the rubric to identify areas of strength and areas of weakness.

Student/Staff Feedback:

  • Summarize what you learned from student/staff feedback collected after your previous summative assessment.

Future Student/Staff Feedback:

Goal Proposal:

The Educator proposes a Student Learning Goal and a Professional Practice Goal

  • Educators in their first year of practice must meet with the evaluator to develop goals.

  • Educators on Self Directed Plans or Educators not in their first year of practice may use team goals.

  • Educators on Directed Growth Plans or Improvement Plans must have goals that address specific standards and indicators identified for improvement in the previous evaluation.

  • The goals should be SMART* goals.

  • Create and Share Self-Assessment form, sign it after Evaluator signs it.

  • Create and Share Goal Setting Form, sign it after Evaluator signs it.
