The plan that is developed is called “Planned Activities” on the TeachPoint Educator Plan Form. There is one box on this form for the “Planned Activities” for each goal.
Planned Activities should include one or more of the following (Evidence of Goal Attainment should be included):
• Educator Activities: Activities to be completed by the educator, such as courses, coaching, books to read, mentoring and induction programs
• Student Activities: Activities to be completed by students.
• District/School Support: Additional Resources available to the educator, such as courses, coaching support.
• Evidence of Goal Attainment: Is a list of the evidence that will be used by the evaluator to determine attainment of Student Learning and Professional Practice goals.
