5.2 Formative Evaluation

Formative Evaluation (April 1)

• Only for 2 year Self-Directed Growth Plans.

• At least 2 weeks before this is due, the educator must provide to the evaluator evidence of family engagement, professional responsibility and growth, and progress towards attaining professional practice and student learning goals. Educator may provide additional evidence.

• Educator and evaluator will meet upon request of either before or after the evaluation is complete.

• Educator may reply in writing within 5 school days of receipt.

• Educator must sign within 5 days of receipt. Signature does not indicate agreement.

• Evaluator may change the activities in the plan or the type of plan based on the new overall rating.
