A yearly physical examination is required for interscholastic participation.  Students planning to participate in athletics are required to obtain their own physical examination prior to participation.  To avoid problems with summer physical appointments, it is suggested that the student-athlete schedule his/her physical in the late spring and submit the results the school before the close of school in June.

The Seymour Physical Form must be completed by a physician and submitted to the school nurse prior to participation.  The cost of the examination is the responsibility of the student/athlete.
