Instructional Setting

Instructional settings are based upon the location of where the child receives the majority of their early intervention services. The following are settings for infants and toddlers (ages birth - 2yr. 11mo.) receiving early interventions services:

Documentation of Important Dates

Written Date: The date that the IFSP is written and sent to the family. This date should match the sent date on the PWN for Initiation of Services. 

Service Plan Meeting Date: The date of the team meeting held to develop this IFSP. For an initial IFSP, this will be your initial IFSP meeting date. For an annual IFSP meeting, this date is the date of the annual IFSP meeting. *If more than one meeting is required, record the date of the first of a series of meetings during which the proposed IFSP was reviewed or developed.

Referral Date: The date the local central point of intake (the district) received the referral. This information is displayed on the initial and any subsequent IFSPs.

Initial IFSP Meeting Date: This is the first IFSP team meeting to develop the initial IFSP. This is the date that concludes the 45-day timeline for Part C evaluations. This information should be displayed on the initial and any subsequent IFSPs.

Timelines

The IFSP must be written and sent to families within 14 calendar days from the date of the IFSP meeting

The annual IFSP meeting must be held on or before the date of previous IFSP meeting (must be within 6 months of the previous periodic review)

Reason(s) if 45-day timeline does not apply or is not met

You must document reasons why the 45-day timeline has not been met. Federal regulations set out two specific situations where the 45-day timeline does not apply: 

If the 45-day timeline does not apply for one of the two exceptions stated above, the district or early intervention service provider must complete the screening, initial evaluation, initial assessment, and initial IFSP meeting as soon as possible after the basis for the exceptions ceases to exist. These exceptions do not constitute a waiver of the 45-day timeline, and the district or early intervention service provider must develop and implement an interim IFSP (to the extent appropriate).
