A family must be given an opportunity to fill out a Family Outcome Survey when an infant/toddler exit services to determine how much early intervention services have helped their family to:

This survey is voluntary by the family and the family responses must be anonymous.

Explanation
The Family Outcomes Survey (Part C Version) is given to families of infants or toddlers with disabilities by their primary service provider. The survey is given to all children who have been served by Part C for six months or more, as the child turns age three or is exiting Part C for any reason. The Minnesota Department of Education relies on responses from families to meet federal reporting requirements. Each state is required to report the percent of families participating in Part C who report that early intervention services have helped their family to:
Know their rights.
Effectively communicate their children's needs.
Help their children develop and learn.
Why Family Outcome Survey?
For families, the goal of early intervention and early childhood special education is to enable them to provide care for their children and have the resources they need to participate in their own desired family and community activities.
Early intervention and early childhood special education also support families in a variety of individualized ways to help them care for their children and have the resources they need to participate in their own desired family and community activities. States collect, analyze, and use data on the percent of families participating in Part C who report that early intervention services have helped.
When to Give the Family Outcomes Survey
This survey shall be given for all children who have been served by Part C (0-3 ECSE) AND who have participated in Part C for 6 months or more as the child turns 3 or is exiting part C for any reason:
The child is turning 3 and is transitioning to services under Part B (3-5).
The child has been determined to no longer be a child with a disability following a reevaluation.
The family has decided to discontinue participating in Part C services.
DO NOT include children who are moving from your district but who will continue Part C services in another MN school district upon relocation.
Note: the list of children whose parents should receive the family outcomes survey is the same as the child for whom a Part C exit COS rating should be done.
Steps for Service Coordinator to Complete
The Part C Service Coordinator will:
Copy/past/type the MARSS number onto the survey on the MDE website in the appropriate language and print
Hand deliver to the parent during a home visit
Explain the importance of this information in overall program support and improvement

Make certain that the complete survey (Side A and Side B) are distributed to families at exit. (Side A is used as part of the family assessment to help identify family priorities.)
