Determining Eligibility for Part C

The members of the IFSP Team discuss their findings and conclusions and determine if there is documentation, data, or other evidence that the child meets the eligibility requirements. See the Evaluation Planning page for more information on determining eligibility for Part C. 

Documentation Eligibility Determinations (initial IFSP only)

Check the box that corresponds to the appropriate Part C eligibility determination and provide any additional information requested, such as rationale for determination. 

If the child is eligible for Part C services, specify which component of the birth through 2 criteria listed below was used. 

Developmental Delay (1.5 standard deviations or more below the mean in at least one developmental area)

The child is experiencing a developmental delay that is demonstrated by a score of 1.5 standard deviations or more below the mean, as measured by the appropriate diagnostic measures and procedures, in one or more of the following areas:

Record the area(s) of development the child has a qualifying score in, the instrument used and the score received (in standard deviations). If eligibility was determined through a review of outside medical or other records, indicate the source/provider in the orange box.

Developmental Delay (diagnosed physical or mental condition or disorder)

The child has a diagnosed physical or mental condition or disorder that has a high probability of resulting in developmental delay regardless of whether the child has a demonstrated need or delay. Record the specific condition(s) and the source of the child’s diagnosis. 

Categorical Disability

The child meets the criteria of one of the disability categories as defined in Minnesota Rules. For more information on categorical eligibility, visit the Assessment page and click on the appropriate disability category.

Select a disability category from drop down menu of all categorical disability areas set out in Minnesota Rules.

Click on the SpEd criteria box to automatically insert criteria for the corresponding disability category. Mark with X's to finish categorical disability eligibility determination. **Note: You may need to reach out to a staff with an appropriate licensure for assistance in filling out the categorical disability criteria (i.e. Teacher of Deaf/Hard of Hearing, Teacher of the Blind/Visually Impaired)

Developmental Delay (informed clinical opinion)

The child's eligibility is established through the application of informed clinical opinion. Informed clinical opinion may be used as an independent basis to establish a child's eligibility under this part even when other instruments do not establish eligibility; however, in no event may informed clinical opinion be used to negate the results of evaluation instruments to establish eligibility.
