Periodic Review
Part C (Birth-Age 3)
Explanation
The purpose of the periodic review is to:
determine the degree to which progress toward achieving the results or outcomes identified in the IFSP is being made and
determine whether modification or revision of the results, outcomes, or early intervention services is necessary.
Periodic review must be documented in writing, even when information has been reported orally (e.g., via phone conference). Documentation resides on each Outcomes page of the IFSP or through the IFSP Progress Report. See information below for examples.
Timelines
A review of the IFSP for a child and the child's family must be conducted every 6 months, or more frequently if conditions warrant.
Therefore, a period of no longer than 6 months can ever lapse between two reviews, whether they are periodic or annual reviews.

Required Team Members
The periodic review may be carried out through a meeting or another means acceptable to the parents and other participants. Required team members include:
The parent or parents of the child
Other family members, as requested by the parent, if feasible to do so
An advocate or person outside of the family, if the parent requests that the person participate
The service coordinator designated by the public agency to be responsible for implementing the IFSP
Note: If conditions warrant, provisions must be made for the participation of other representatives from the IFSP team.
Documenting Periodic Review
The following documents are (1) sent to the parent/guardian and (2) filed in the child's special education paperwork:
IFSP Outcomes pages (see screenshot below)
Progress Monitoring graphs, checklists, monitoring sheets
Prior Written Notice outlining summary of periodic review and any proposed revisions to the IFSP
Notice of Team Meeting (if the periodic review will occur through a meeting)
Some service coordinators also send the "Understanding Your Child's Graphs" cover sheet to parents because it can help parents to interpret the graphs.
Steps to Complete the Periodic Review through the Outcomes Pages
Send the family a Notice of Team Meeting if the periodic review will occur through a formal meeting
Indicate the date of the periodic review on the Student Information page of the IFSP
Click on the "Child and Family Outcomes" tab under the IFSP in SpEd Forms (see screenshot).
Navigate through each outcome and:
Enter date periodic review conducted on
Review child and/or family's ongoing assessment data and progress monitoring data.
Describe the child/family's progress toward the measurable result or outcome. This includes quantitative data ("Brenda has been able to maintain a sitting position for 1 minute.") and whether or not the student is expected to meet their goal ("If Brenda continues making progress at the current rate, it is expected that she will accomplish this outcome by the time of the annual IFSP.")
Select appropriate checkbox(es) (Accomplished, Continue, Discontinue, Continue with the Following Modifications or Revisions) and clarify any modifications or revisions that will be proposed to the IFSP.
