Explanation

The purpose of the periodic review is to:

Periodic review must be documented in writing, even when information has been reported orally (e.g., via phone conference). Documentation resides on each Outcomes page of the IFSP or through the IFSP Progress Report. See information below for examples.

Timelines

A review of the IFSP for a child and the child's family must be conducted every 6 months, or more frequently if conditions warrant. 

Therefore, a period of no longer than 6 months can ever lapse between two reviews, whether they are periodic or annual reviews.

Required Team Members

The periodic review may be carried out through a meeting or another means acceptable to the parents and other participants. Required team members include:

Documenting Periodic Review

The following documents are (1) sent to the parent/guardian and (2) filed in the child's special education paperwork:

Some service coordinators also send the "Understanding Your Child's Graphs" cover sheet to parents because it can help parents to interpret the graphs.
