In order to progress through the application you MUST have this information:
Your Social Insurance Number (SIN) and Saskatchewan Health Services Number (HSN);
For full-time students, if you are dependent upon your parents, you will need your parents' SIN, HSN, dates of birth and information about their marital status including the date they were married, or the date separated if you are dependent on one parent;
If you are married, you will need your spouse's SIN, HSN, birth date, and information about what they will be doing while you are in school; and
If you have dependent children, you will need the HSN and birth date for each child.
If you are missing anything, you won't be able to complete it. The application only takes about 1/2 hour and you will have two forms that you need to print, sign and have parents/guardians sign to mail/fax in after.
Post-secondary students who graduate from certain programs in high-demand occupations may be eligible to receive up to $20,000 in student loan forgiveness on the Saskatchewan portion of their student loan for working in designated communities across Saskatchewan.
