Foster Care and McKinney-Vento Mandatory Forms:
Sample CPS 2085 Forms (this form must be provided to the campus by the CPS or Foster Parents)
Foster Care / McKinney-Vento Enrollment Conference Form (this form must be completed for every Foster Care and McKinney-Vento student within 10 days of enrollment/identification)
Socioeconomic Forms:
The preferred method for the collecting Economic Disadvantaged or Socioeconomic data is to have parents complete the Meal Application or Socioeconomic Form online using their Skyward Parent Portal.
NOTE: If the below paper forms are collected, the data provided on the form must be manually entered into Skyward by the campus in order for student data to be updated.
