In keeping with NCLEX style, utilize these terms:
Client – refers to an individual, family, or group that includes significant others. DO NOT make up names for the client and do not use the term ‘patient’
Nurse – refers to the registered nurse. DO NOT name the nurse or use gender specific terms. Simply state as an example: “The nurse is providing care for an elderly client.”
Prescription – pertains to orders, RX, interventions, remedies or treatments ordered or directed by an authorized health care provider.
Health care provider – used to make reference to a physician, APRN, PA, etc. (DO NOT use 'physician', 'doctor', etc.)
UAP – unlicensed assistive personnel who is an aide, tech, CNA, PTC, etc.
Use: ‘The nurse’ not ‘You’ are the nurse or ‘You’ are taking care of a client.
Use of race, religion, gender, sex, age (based on developmental age references; infant, toddler, preschooler, and school age etc.) ONLY as needed to have the learner accurately answer the question. Do not use ages. Please refer to developmental stage across the continuum from infant to elder or aging population.
Write “client sustained a motor vehicle accident” rather than “client with a motor vehicle accident (MVA).”
Use “A client with diabetes” not “A diabetic client”.
Use: central venous access device (CVAD) (DO NOT use ‘central line’).
Avoid use of brand names such as ‘foley’ catheter.
