Using the Google Forms Template, you can create and customize your own Client Engagement Form
Go to Google Forms at https://docs.google.com/forms or click on the Google Apps hamburger in the upper right corner of Chrome.
At the top of the Google Forms page there is a Templates section where you will see the Client Engagement Template. If you do not see it, click the Template Gallery.
Click the Client Engagement Template and rename it with your name for easy identification.
Modify the form to fit your personality and style under the Questions tab and by clicking on the individual question. Remember the red star means the question is required.
Under the Responses tab, be sure to click the three dots to the right of sheets and have it notify you when responses are received.
Under Settings and Presentation, you can add a custom confirmation message that includes a link to your calendar to schedule a meeting.
Lastly, publish your form and make sure it's accessible to anyone with the link.
