Health
As a professional in health, you will most likely be involved with children who have ALN. You may provide intervention, complete assessments, or simply refer a case to Powys County Council because you believe the child you are working with may have additional learning needs.
The Additional Learning Needs Code for Wales 2021 sets out duties for the local health board. These duties include:
to consider whether there is a relevant treatment or service likely to be of benefit in addressing the ALN of a child or young person;
securing the ALP (that is, the relevant treatment or service) described in an IDP as provision to be secured by the Local Health Board (including taking reasonable steps to secure its provision in Welsh where this is required);
participation in reviews of IDPs in which a relevant treatment or service is set out as ALP to be secured by the Local Health Board;
requests from local authorities to the Local Health Board for information or other help, which the former requires for the purpose of exercising its ALN functions;
informing parents and local authorities of the Local Health Board’s view that a child under compulsory school age probably has ALN.

Powys County Council uses the Tyfu platform to store IDPs, store documents, and process referrals. To make it easier for health professionals to fulfil their duties under the ALN code, a set of systems have been put in place. Below, you will find information on how to access Tyfu profiles to be able to upload reports and documents, information for health visitors who want to notify the Local Authority of possible ALN, the process for writing health ALPs, and attending review meetings.
Accessing Tyfu profiles
There are a few circumstances where you as a health professional may be invited to the Tyfu profile of a child.

The invitation to access a child’s profile will go directly to your email address. You will also be sent a 6 digit code for the specific child. Please see the invitation guide for step-by-step instructions on how to access the profile.
Reports and documents can then be uploaded to the ‘plans and reports’ tab.
Early Years Health Visitors
To notify the Early Years ALN team that a child that you have visited may have ALN, please use the Microsoft form which can be found here: PTHB Notification to PCC (office.com)

Health ALPs
Some pupils who have ALN may need ALPs that are either written by health professionals and delivered by school staff or delivered entirely by health professionals. When this is the case, schools should refer the child to the Local Authority via Tyfu. The Tyfu team will then notify the relevant service of the request and the school will invite the professional to the Tyfu profile. The ALPs can then be written in the Additional Learning Provision tab on Tyfu by the relevant health professional.
Attending review meetings
Health input may be necessary to write and review IDPs. If a health professional is required to attend a Joint Assessment Meeting (JAM) or a review meeting, an invitation will be sent to the professional and access may be given to the child’s Tyfu profile. If you are unable to attend the meeting, you may be required to send in a report instead so that a professional health perspective can be included in the final IDP.
Out of county schools
Powys County Council uses the Tyfu platform to host all ALN and PEP information, which includes One Page Profiles, Universal Learning Provision Plans, and Individual Development Plans (IDPs). If a child has a Local Authority Maintained IDP but attends school in an authority outside of Powys, the IDP will be hosted on Tyfu. As a school, you will be able to access this information through an invitation to view the pupil’s Tyfu profile. The invitation to access a child’s profile will go directly to your email address.
