The Department of Labor and Industry mandates certain reporting requirements pertaining to an employee fatality, inpatient hospitalization, amputation, or the loss of an eye during the performance of job duties and/or on school division property.

The County Office of Emergency Management & Workplace Safety (EMWS) is available to assist with reporting, however school administrators must make the required report to Virginia Occupational Safety & Health (VOSH).
The Departments of Risk Management and Human Resources must be notified as well.
Risk Management would prefer that contact be made with its office prior to making the notification to VOSH, however notification to VOSH should not be delayed if immediate contact with Risk Management is not possible.
