Medications at School

What you need to know...

All students needing a medication at school must have a Medication Authorization form filled out.


IMPORTANT NOTE: No student should transport medications to and from school, this is the responsibility of an adult.


Prescription medications requires a doctor’s signature AND a parent or guardian signature on the medication authorization form, with the exception of inhalers where an inhaler sticker may be placed on the form, and the parent/guardian still signs.


Nonprescription medications do not require a physician’s signature but a form must be filled out & signed be a parent or guardian.


Students may carry their Inhalers and/or EpiPens with them with the approval of their school nurse (a form is still needed).


Please note: Medication authorizations must be filled out EVERY new school year and doctor’s notes do not carry over year to year.
