There are no mandated requirements by the state of Illinois for 8th grade. However, If your student gets a yearly physical done AFTER the start of 8th grade, this can count as their 9th grade physical if completed on the state of Illinois form.

It is recommended to get your student(s) a yearly well check and submit a copy of that physical to your school nurse.

**If your student(s) plans to participate in sports, an IHSA sports physical OR state of Illinois physical form with the 'interscholastic sports participation' box checked, must be completed and turned into your school nurse.

Please have all documents completed and signed by the appropriate physician and turned in to your school nurse OR submit copies onto Skyward.
