9th GRADE (Freshmen year) Mandated Requirements

The State of Illinois School Code has established mandated health requirements for entry into ninth grade. Listed below are the mandated requirements that must be completed and submitted prior to the beginning of school in August. Failure to comply with these requirements will result in exclusion from school. If you have any questions, please feel free to call your school nurse.


School physical: Must be on the State of Illinois form with the student’s up to date immunizations, medical health history completed and signed by a parent. The physical exam section must be signed and dated by the physician and be completed within one year to the start of school. Height, weight, & blood pressure must be on the form for compliance.

**A sports (IHSA) physical is the incorrect form needed for 9th grade.**


Dental exam: All 9th grade students are required to have a dental exam. This requirement is due by May 15th of your child’s 9th grade year.

Immunization Requirements:

There are no new immunizations required to begin 9th grade. The following are required immunizations that your child should already have received. If for some reason your child is not compliant with these immunizations please have them updated prior to the beginning of school in August.

  • Tetanus-Diphtheria-acellular Pertussis (TdaP) booster vaccine: All students entering ninth grade must provide proof of having received the Tdap vaccination.

  • Measles, Mumps & Rubella: All students entering ninth grade must show proof of having had two doses of Rubella and Mumps or show proof of immunity described in Section 665.250(c). This is a portion of the MMR. If your child has 2 MMR’s at the appropriate intervals they are compliant with this mandate.

  • Hepatitis B: All students entering ninth grade must show proof of having received three doses of hepatitis B vaccine as directed by the state of Illinois guidelines or other proof of immunity described in Section 665.250(f).

  • Varicella: All students entering ninth grade must show proof of having received 2 doses of the varicella vaccine or proof of prior varicella disease as described in Section 665.250 (g).

  • Meningococcal (MCV): All students entering ninth grade must show proof of having had one dose of the meningococcal conjugate vaccination (MCV) on or after 11 years of age.


Religious or Medical Objection: any parent that is requesting religious or medical exemption from immunizations and/or health examinations MUST present to the school nurse a Certificate of Religious or Medical Exemption form completed and signed by their physician by the first day of school.

**DO NOT wait until the last minute to schedule your student(s) appointments.**

Please have all requirements completed and signed by the appropriate physician and turned in on or before the first day of school OR submit copies onto Skyward.
