Please ask your child’s physician for an updated Asthma Action Plan at every yearly visit. A copy of this should be turned into the school nurse each school year.

A Medication Authorization form is linked here. The parent/guardian may attach one of the inhaler prescription stickers to the form & must sign the medication form.

Discuss the needs of your student with their school nurse.
