12th GRADE (Senior Year) Mandated Requirements

The State of Illinois School Code has established mandated health requirements for entry into twelfth grade. Listed below are the mandated requirements that must be completed and submitted prior to the beginning of school in August. Failure to comply with these requirements will result in exclusion from school. If you have any questions, please feel free to call your school nurse.


Immunization Requirements: All students entering 12th grade will be required to show proof of receiving two doses of meningococcal conjugate vaccination (MCV) unless the first dose was administered after 16 years of age. In this case, only one dose after 16 years of age is required.


Religious or Medical Objection: any parent that is requesting religious or medical exemption from immunizations and/or health examinations MUST present to the school nurse a Certificate of Religious or Medical Exemption form completed and signed by their physician by the first day of school.


It is recommended to get your student(s) a yearly well check and submit a copy of that physical to your school nurse.

**If your student(s) plans to participate in sports, an IHSA sports physical must be completed and turned into your school nurse.


Please have all documents completed and signed by the appropriate physician and turned in on or before the first day of school OR submit copies onto Skyward.
