There are no mandated requirements by the state of Illinois for 7th grade.

It is recommended to get your student(s) a yearly well check and submit a copy of that physical to your school nurse.

**If your student(s) plans to participate in sports, an IHSA sports physical must be completed and turned into your school nurse.

Please have all documents completed and signed by the appropriate physician and turned in OR submit copies onto Skyward.
