Ask your child’s physician for an updated Diabetes Medical Management Plan at every yearly visit. A copy of this should be turned into your child’s building school nurse each school year.

A Medication Authorization form is linked here. The physician & parent/guardian must sign one for each prescription medication. Parent signature required for each over the counter medication. Your school nurse should have Emergency medications on hand for your student.

Glucose Monitoring

Carbohydrate Counting:

People with Type 1 (insulin-dependent) diabetes must match their carbohydrate intake to their insulin dose. To get the best blood sugar result, carbohydrate counts must be accurate. Carbohydrates are counted in grams – even a few grams can make a difference in one’s blood glucose level.

For people with type 2 diabetes, being aware of the carb content of food can help with diabetes control. Increasingly, people with type 2 diabetes who use insulin are being taught about carb counting, both on courses or in consultation with their health care professional.
