There are no mandated requirements by the state of Illinois for 4th grade.

It is recommended to get your student(s) a yearly well check and submit a copy of that physical to your school nurse.

Please have all documents completed and signed by the appropriate physician and turned in to your school nurse OR submit copies onto Skyward.
