Dental Exam- The State of Illinois requires all students entering kindergarten for the first time to have a dental exam. This must be performed by a licensed dentist.

For 2nd grade, 6th grade, and 9th grade: The dental examination must be performed by a licensed dentist within 18 months prior to May 15th of the school year.

Turn in the completed form to your school nurse.

All dentist offices carry this form.
