5th Grade Requirements
There are no mandated requirements by the state of Illinois for 5th grade. However, If your student gets a yearly physical done AFTER the start of 5th grade, this can count as their 6th grade physical if completed on the state of Illinois form.

It is recommended to get your student(s) a yearly well check and submit a copy of that physical to your school nurse.
