All new and transfer students are required to submit the required medical forms within 30 days of starting in District #228.
All New and Transfer students attending an Illinois school for the first time, in addition to their grade level requirements your student(s) must have:
School Physical- A school physical is required regardless of grade level when entering for the first time to an Illinois school and must be on a state of Illinois form. This physical must have been within the last year to meet requirements.
Illinois school to Illinois school transfer can submit their last grade level required physical (Kindergarten, 6th grade, 9th grade).
**A sports (IHSA) physical is the incorrect form needed for new or transfer students.**
Vision Exam- All out of state transfer students entering an Illinois school for the first time must have a vision exam. The vision exam must be completed by an ophthalmologist or an optometrist within 1 year of the transfer into the Illinois school.
Immunizations- All immunizations, including updates, should be complete per Illinois grade level requirements.
Religious or Medical Objection: any parent that is requesting religious or medical exemption from immunizations and/or health examinations MUST present to the school nurse a Certificate of Religious or Medical Exemption form completed and signed by their physician by the first day of attending school.
If you are transferring in from outside of Illinois, please reach out to your building nurse to check requirements.
Please have all requirements completed and signed by the appropriate physician and turned in within 30 days of enrollment OR submit copies onto Skyward to avoid medical exclusion from school to take place.
