All students needing a medication at school must have a Medication Authorization form filled out and turned into the school nurse.

IMPORTANT NOTE: No student should transport medications to and from school, this is the responsibility of an adult.

**Prescription medications require a doctor's signature on this form of a sign note containing orders for the medication.
