Religious Objection: any parent that is requesting religious exemption from immunizations and/or health examinations MUST present to the school nurse a Certificate of ReligiousExemption form completed and signed by their physician by the first day of school.

All physician's offices carry this form.

This cannot be signed by a chiropractor.
