Pupil Personnel Services Credential
Current Students
The California Commission on Teacher Credentialing (CTC) establishes and oversees the requirements of the Pupil Personnel Services Credential (PPSC) in School Social Work and Child Welfare and Attendance. The CTC requires that as part of your field practicum experience, you document your hours, the activities related to school social work and child welfare and attendance, your supervisor’s confirmation of your hours, and your supervisor’s evaluation of your learning goals.
This site contains forms required for documentation of PPSC activities, performance evaluation, and accumulation of hours supervised by your qualified PPSC supervisor.

Requirements for MSW Embedded PPSC Students and Post-MSW PPSC Students
(MSW Embedded students complete BOTH MSW practicum forms and PPSC forms in IPT to fulfill the MSW degree and PPS credential requirements. See table below for details and differences in requirements between MSW Embedded and Post-MSW PPSC students))
Please Note: All current MSW students continue to complete all MSW Clinical field practicum forms in IPT (Clinical Time Log, Clinical Learning Agreement & CSE, and Agency Orientation Checklist) in addition to the PPSC forms (also available in IPT) listed below.

Current MSW PPSC SSW-CWA students:
(Regular MSW Clinical field practicum forms in IPT AND the following PPSC forms in IPT are required - DOWNLOAD only for reference)
SSW-CWA forms:
● PPSC SSW-CWA Learning Agreement & CSE (DUE: October 6, 2023 with MSW Learning Agreement; CSE DUE: End of Fall semester and end of Spring semester)
● PPSC SSW-CWA Field Practicum Activities Checklist Sign Off (DUE: End of Spring Semester)
● PPSC Field Practicum Time Log (DUE: End of Fall and end of Spring Semester)
● PPSC SSW-CWA Standard 4 Sign Off (DUE: End of Spring Semester)
● Requirements for Process Recordings (DUE: To field practicum instructor/supervisor) & Process Recording Log (DUE: In IPT with Field Practicum Instructor signatures)
Minimum ONE (1) IEP and ONE (1) SARB PR are required per academic year. (In the event that attendance to an IEP or SARB/SST/SART meeting is not possible, student and field practicum instructor may discuss alternative process recordings.)
Fall Semester (6 total): Along with the above IEP and SARB requirements, minimum FOUR (4) MICRO PRs required. Remaining PRs may be of any form.
Spring Semester (8 total): Along with the above IEP and SARB requirements, minimum FIVE (5) MICRO PRs required. Remaining PRs may be of any form.
Post MSW PPSC SSW-CWA students:
(The Agency Orientation Checklist, Mid-Semester Progress Report, and Process Recording Log in IPT AND the following PPSC forms in IPT are required)
SSW-CWA forms (DOWNLOAD only for reference):
● PPSC SSW-CWA Learning Agreement & CSE (DUE: End of Fall semester and end of Spring semester)
● PPSC SSW-CWA Field Practicum Activities Checklist Sign Off (DUE: End of Spring Semester)
● PPSC Field Practicum Time Log (DUE: End of Fall and end of Spring Semester)
● PPSC SSW-CWA Standard 4 Sign Off (DUE: End of Spring Semester)
● Requirements for Process Recordings (DUE: To field practicum instructor/supervisor) & Process Recording Log (DUE: In IPT with Field Practicum Instructor signatures)

Spring Semester (4 total): Minimum ONE (1) IEP required. The remaining three may be on SARB/SST/SART/504 Plan meetings.
