MSW Practicum Forms
Students should complete all forms in the InPlace tracking system (except for Process Recordings).
With the exception of the Process Recordings, all forms on this page are for information purposes only.
Please contact your APU Practicum Liaison for questions or problems accessing the forms or links.

Requirements for MSW Students:
Your Practicum Instructor's signatures in IPT, 2) submitting no more than one process recording per week, 3) mid-semester submissions (3 for fall and 4 for spring by mid-semester), 4) along with the following criteria are required to receive full credit (See syllabus for details).
Fall Semester: Minimum TOTAL of SIX (6) process recordings (PR) required. No more than one process recording per week.
Spring Semester: Minimum TOTAL of EIGHT (8) process recordings required. No more than one process recording per week.
MSW Generalist Foundation Year Students
Fall Semester: Minimum FOUR (4) MICRO and ONE (1) MACRO PRs required. Remaining PR may be of any form.
Spring Semester: Minimum FIVE (5) MICRO and ONE (1) MACRO PRs are required. Remaining PRs may be of any form.
MSW Specialization Year Students
NEW for 2025-26: One (1) live supervision/reflection form may be submitted in lieu of two (2) MICRO process recordings per semester (this is in addition to the fall live supervision assignment)
Clinical Students: Minimum FOUR (4) MICRO PRs required for FALL & SIX (6) MICRO PRs required for SPRING semesters. Remaining PRs may be of any form.
Macro Community Students: Minimum FOUR (4) MACRO PRs required for FALL & SIX (6) MACRO PRs required for SPRING semesters. (One MEETING form in lieu of one MACRO form allowed). Remaining PRs may be of any form.
MSW PPSC Students
Minimum ONE (1) IEP and ONE (1) SARB PR are required per academic year. (In the event that attendance to an IEP or SARB/SST/SART meeting is not possible, student and practicum instructor may discuss alternative process recordings. It is the student’s responsibility to communicate with their Practicum Instructor and/or Faculty Liaison to discuss ways to advocate for these opportunities in their placements.)
Fall Semester (6 Total): Along with the above IEP and SARB requirements, minimum FOUR (4) MICRO PRs required. Remaining PRs may be of any form.
Spring Semester (8 Total): Along with the above IEP and SARB requirements, minimum FIVE (5) MICRO PRs required. Remaining PRs may be of any form.
Post MSW PPSC Students
Minimum ONE (1) IEP and ONE (1) SARB PR are required per academic year. Spring Semester (4 Total): Minimum ONE (1) IEP required. The remaining three may be on SARB/SST/SART/504 Plan meetings.
