Central Connections provides case management for individuals in Cole County with intellectual and developmental disabilities.
Eligibility: Individuals must have a developmental disability that began before the age of 22, and is likely to continue indefinitely. The disability results in substantial limitations with two or more of the following: living and working independently; written or verbal communication, self-direction, learning, mobility, or self-care.

Residential facilities provide 24/7 support and assistance to adults with developmental disabilities in all activities of daily care. Respite services provider for temporary and short term oversight of the safety, well-being and daily living care needs of a person with developmental disabilities ages thirteen and older.

HHC works with local hospitals, physical therapists, social workers, and other health care providers to identify individuals struggling to live independently in their own homes.
HHC loans equipment such as hospital beds, bedside toilets, wheelchairs, and shower benches to these individuals at no cost. All items are donated by members of our community.
Volunteers assist with shopping, errands and doctor’s visits for seniors.

CLAIM stands for “Community Leaders Assisting the Insured of Missouri.” We have been Missouri’s official State Health Insurance Assistance Program (SHIP) since 1993. All of our services are free, unbiased, and confidential. We are not an insurance agency. We do not sell anything, and CLAIM counselors do not ask for or accept money.
CLAIM has more than 300 volunteer counselors throughout Missouri providing local Medicare counseling in their communities. Volunteers receive extensive training to become certified Medicare counselors, and continue to receive updates and continuing education on a regular basis.
Calling 1-800-390-3330: Our friendly Contact Center staff will take your name and basic information. Then they will relay that information to one of our local community partners. Within days, a volunteer counselor calls back to set up an appointment or to discuss your Medicare issue by phone. The helpline is open from 9 a.m. to 4 p.m., Monday through Friday, excluding federal holidays.

Voc Rehab specializes in employment and training services that can assist you in becoming employed. A VR counselor will determine your eligibility for services. To be eligible, you need to have a physical or mental impairment that causes problems with working and need VR services to be successfully employed. Once eligible, you will work with a counselor who will help you develop a plan for your rehabilitation.

New Horizons provides people with support so they can learn to manage the impact that mental illness has on their daily life. We find that people with mental illness and substance use disorders often need support with finding a psychiatrist, obtaining medication, and learning to manage their symptoms; finding and keeping housing; finding and keeping a job; and learning to manage their time in productive ways. We assist people with severe and persistent mental illnesses and co-occurring substance use disorders by providing quality psychiatric rehabilitation services. We offer people hope, kindness, and respect for their process of recovery, while helping them effectively manage their lives.

The Special Learning Center provides comprehensive, early intervention services for children with developmental delays and disabilities. Children served in the Special Learning Center program have delays ranging from mild to severe. Children without delays also attend the Special Learning Center and serve as friends and role models to their peers. In this program, our classrooms offer a warm, caring environment where individualized and appropriate learning activities for each child are offered. Students are grouped by developmental and age levels into early childhood classes. Sign language classes, physical therapy, speech therapy, and occupational therapy are offered on site and in the outlying school district. We also have a childcare center called Crayon Kids. Student/teacher ratios are kept to a minimum to maximize each student’s progress and help prepare students for entry into public school or their next educational placement.
