Data Collection Opt Out Form

As part of my job as a college adviser, I collect data on student's plans for college. The data that I collect helps me keep track of each student's post grad plans and helps the organization that I work for assess the impact of the program at SSS. This collected data is limited. I only note names, school email addresses, graduation year and any notes from the interactions that we have as students work through the college application process.

Students have the option of opting out of this data collection. This means that I won't keep any handwritten or electronic notes about anything that we discuss. I can and will continue to advise you about college and other options if you choose to opt-out. More information on the opt-out process can be found in the document below.
