Diagnoses: Autism, ADHD, Any sensory integration or processing difficulties
Setting: school-based, pediatric outpatient,
help evaluate a child's sensory processing patterns in the context of home, school, and community-based activities
Auditory processing
Visual processing
Touch processing
Movement processing
Body position processing
Oral sensory
Conduct associated with sensory processing
Social-emotional responses associated with sensory processing
Attentional responses associated with sensory processing
Time: 35 mins (15-20:administration/15 mins scoring)
Group/Individual: Individual
Subtests: 9 subtests (86 questions total) based on 4 quadrants of sensory processing
Instructions: After completing the demographic information above, the caregiver will read these instructions and proceed with the following questions: "The pages that follow contain statements that describe how children may act. Please read each phrase and select the option that best describes how often your child shows these behaviors. Please mark one option for every statement. Use the guidelines to mark your response:"
When presented with the opportunity, my child...
Almost always - 90% or more of the time
Frequently - 75% of the time
Half the time - 50% of the time
Occasionally - 25% of the time
Almost never - 10% or less of the time
Does not apply - have not observed the behavior or believe it does not apply
Materials: Manual, answer sheet
User Qualifications: Paid training is required
How to Score:
For each sensory or behavioral subsection of the Caregiver Questionnaire, the scores (0-5) will be totaled
For each question, there is a corresponding quadrant, and each score reported on the Caregiver Questionnaire will be transferred into the corresponding column in the Quadrant Grid
Once all scores are transferred, the sum of all the scores will create the Quadrant Raw Score Total for each sensory processing quadrant
Seeking, Avoiding, Sensitivity, Registration (or no quadrant)
The Quadrant Raw Score Total from the Quadrant Grid and Raw Score Totals from the sensory & behavior sections of the Caregiver Questionnaire will be transferred to the corresponding Raw Score Totals Column
The totals are plotted by marking an X in the appropriate classification column (e.g. Less than others, more than others, just like the majority of others) to fall within the normal curve
Score Indications:
Scores one standard deviation from the mean (+/- 1) are considered "more than others" or "less than others"
Scores two standard deviations or more from the mean (+/- 2) are expressed as "much more than others" or "much less than others", respectively
Scores that fall within the mean range are considered "just like the majority of others"
Reliability:
Test-Retest: Caregiver (.83-.97) & School (.66-.93)
Inter-rater: Caregiver (.70-.80) & Teacher (.70-.90)
Validity:
Convergent: Excellent - Spearman’s Rho = 0.86, p < 0.01
Content: 75% agreement
Strengths:
Available in Spanish
Quick and easy for caregivers or teachers to complete
Covers a broad range of sensory processing patterns (e.g., seeking, avoiding, sensitivity, registration).
Helps guide individualized intervention planning in home, school, or therapy.
Based on real-world behaviors in natural environments.
Supports collaboration between parents, educators, and therapists.
Weaknesses:
Subjective – relies on caregiver/teacher perception, which may be biased or inconsistent.
Does not directly measure performance or sensory thresholds in clinical settings.
May be less sensitive to subtle changes over short-term interventions.
Lacks direct link to functional outcomes (e.g., ADLs, school tasks).
