Coaches:

At the beginning of each week be sure to email Scott Samuelson at scott.samuelson82@gmail.com to confirm your home games (time and location) for the week
Bring 2 blank copies of this form to each game, one for each official. The Town is using this as the Invoice to pay our refs, please submit after each game so our Ref's get paid in a timely manner. Email completed forms after each game to:
