LifeMoves has the expertise to rapidly return people to stable housing, but a solution to homelessness requires all of us. We need your help to confront the homelessness crisis in our community.
The LifeMoves model leverages three integrated programs designed for long-term success. Our model is personalized to holistically serve the broad needs of our diverse client population – Veterans, families, single adults, the LGBTQ+ community, aging seniors, and more.
SERVICES:
Residential Services: LifeMoves Residential Services Programs provide interim shelter along with food, clothing, household necessities, case management, and supportive services to help homeless families rapidly return to stable housing.
Supportive Services: LifeMoves Supportive Services Programs aim to remove any barriers that a family may face in their efforts to return to self-sufficiency. Intensive case management and customized support services.
Community Outreach: As the homelessness crisis continues to grow and evolve, so do our clients. In order to best support this fluid landscape, LifeMoves has implemented a variety of programs to serve our clients wherever they are as well as prevention programs for at-risk children.

EMERGENCY ASSISTANCE NETWORK
LifeMoves participates in the Santa Clara County Emergency Assistance Network, providing one-time emergency financial assistance to those in need.
We serve the following zip codes: 94301, 94303, 94304, 94305, 94306, 95035, 95131, 95133 and 95134.
