CHILDREN‘S HEALTH INITIATIVE

The Children’s Health Initiative exists in Santa Clara County to provide free or low-cost medical benefits for you and/or your children.

THE PROGRAMS INCLUDE:

• Full medical, including prescriptions

• Doctor’s visits

• Prevention Services (well-baby visits)

• Health Education

• Dental Care

• Vision Services

• Alcohol and Substance Abuse Care

• Mental Health

MEDI-CAL

All children up to age 19 are eligible to Medi-Cal regardless of their citizenship or immigration status as long as they meet all other eligibility requirements. Individuals receiving Public Assistance (PA) receive zero share-of-cost Medi-Cal during the months in which they are cash eligible.

Talk to someone who speaks your language to determine which plan your child qualifies for. You can be referred to a certified application assistor in your neighborhood. A Certified Application Assistor will personally Help you determine the right plan and complete an application.
